Ten breakthroughs between 1796 and 2020 that most changed the course of medicine, selected by their actual impact on life expectancy and disease burden, spanning prevention, diagnosis and treatment, with first clinical use and key figures.
The first four entries on this list did most of the work of pushing human life expectancy from 40 to 73. Vaccination, anesthesia, antisepsis and antibiotics defeated infection — the leading killer throughout human history. Jenner prevented smallpox in 1796 without knowing viruses existed; 180 years later smallpox became the only infectious disease humanity has deliberately eradicated.
The middle of the list marks medicine’s shift from surgical craft to molecular science: Röntgen’s X-rays (1895) let doctors see inside the body without cutting it open, and the DNA double helix (1953) moved the definition of disease from organs to molecules. We excluded incremental drug improvements and kept only paradigm-creating breakthroughs — insulin stands for hormone replacement as a category, and mRNA vaccines (2020) turned vaccine development from cultivation into code.

|---|---|---|---|---|---|
| 1 | Vaccination | 1796 | Edward Jenner | Prevention | Founded active immunization; smallpox remains the only eradicated infectious disease |
| 2 | Surgical anesthesia | 1846 |
